Dr. David A. Fields, MD
Dr. Fields is a graduate from Albany Medical College (1996) in Albany, New York. He successfully completed residency in Family Medicine at the University of Maryland Medical System in Baltimore, MD in 1999. He has faithfully delivered medical care in Loganville GA for more than 20 years. He treats individuals and families holistically. Dr. Fields enjoys spending time with his family, traveling, and singing.
Melissa A. Smith, CPNP-PMHS

Melissa Smith earned her undergraduate at Michigan State University with a bachelors of science in nursing. She then earned her graduate degree from Augusta University with a Masters of Science in Nursing and her certification as a Pediatric Nurse Practitioner. Melissa has over 18 years years clinical experience in emergency room, intensive care, and primary care.
She has received her board certification as a pediatric mental health specialist. She is experienced in early assessment and interventions for common mental health issues seen in primary care.
Lynsey Rank, MSN, APRN, FNP-C
Lynsey Rank has graduated with her bachelors of science in nursing from University of Texas and her masters in family medicine from Chamberlain University. She began working in a hospital in 2010.Since then, she has obtained 10 years of experience in the ER and 5 years in surgery working in the operating room. During the Covid- 19 crisis she was able to assist by working directly with the health department for a year.
